Licensing
- Applications for establishments are available at City Hall in the Clerk/Administration Office or by saving the downloadable files below.
- Class B license applications require an Alcohol Beverage Individual Questionnaire for each partner of a partnership, officer, director, member, manager, and/or agent.
- Class B license applications also require an Alcohol Beverage Appointment Of Agent.
- Class B license applications may require a Business Operation Plan and Security Plan to be submitted.
- Original Alcohol License Fees (cash or checks payable to City of Port Washington):
- $100.00 Class A Beer
- $500.00 Class A Liquor
- $100.00 Class B Beer
- $500.00 Class B Liquor
- $100.00 Class C Wine
- $30.00 Publication Fee
- $10.00 Background Check Fee for each Individual Questionnaire

- The Police Department will conduct background checks of applicants.
- Applications must be published by the City in the Ozaukee Press two weeks prior to consideration.
- Final approval of an Alcohol License will be considered by the Finance & License Committee and Common Council, which meet the 1st and 3rd Tuesday of each month. (July: one (1) meeting is held on the First Tuesday)

Below please find all 2025-2026 License Applications for Renewal of Alcohol License, Cigarette/Tobacco Product License, Annual Cabaret License, Annual Amusement License, Sidewalk Dining Table and Chair License that you may have previously filed with the City of Port Washington.
After you have fully completed all your applications, please sign, date and submit with required documents and fees to the City of Port Washington, at the following address:

If you have questions or if you would like to meet with the City Clerk, please contact City Hall, Monday - Friday 8:00 a.m. - 4:00 p.m., at 262-284-5585 and request a meeting be scheduled for you.
Application
- Applications are available at City Hall or by download here.
- Include the following when you submit your application:
- Form AB-220, Temporary Alcohol Beverage License
- Form AB-100, Alcohol Beverage Individual Questionnaire for all officers, directors, and agent of the nonprofit organization
- Form AB-101, Alcohol Beverage Appointment of Agent
- Payment for license and background check fees, as required.
- Application Fee: $10.00 plus Background Check Fee of $10.00 for each Individual Questionnaire
Submission
- Submit the completed application with the required fees to the City Clerk.
- License applications must be filed with the City Clerk at least 15 days before they can be approved by the governing body.
- The Police Department will conduct background checks of applicants.
- City Clerk grants final approval for applications and issues licenses.

Cigarette and Tobacco Products

- Application fee: $20.00 per machine (cash or checks made payable to City of Port Washington).
- Final approval of license will be done in the Clerk/Administration Office.
- Annual Establishment Cabaret License Applications are available at City Hall in the Clerk/Administration Office or download here.
- Annual Establishment Cabaret License Application Fee: $100.00 (cash or checks made payable to City of Port Washington).
- Final approval of Annual Establishment Cabaret Licenses will be considered by both the Finance & License Committee and Common Council, which meet the 1st and 3rd Tuesday of each month.
